I understand that all visits will incur a 'Visit Charge', the price of which will depend on the location of my horse. I have been informed that some products and services may be required to be paid in advance. I understand and agree that all other costs incurred while my horse(s) are in the care of Page & Gunstone Equine Vets are payable within 30days of invoicing. I have been advised that medicines may be appropriate for the treatment of my horse(s) which are not authorised for the particular use proposed. I have been informed that such medicines have not passed the regulatory assessments for safety, quality and efficacy, for the particular use proposed, but that their use is always justified (both clinically and legally). I have been made aware of, and accept, that there may be unknown side-effects associated with the use of these medicines in my horse(s), and I consent to their use. In accordance with EU General Data Protection Regulation May 2018 we would like to inform you that your details and the details of your horse(s) will be stored on our practice management system to allow us to function as a veterinary surgery. Your details will be kept safe and secure, only used by us, or those who work for us. We ask you to consent to the practice contacting you in the future (by email, text, or phone call) with regards to any matters relating to your horse(s) wellbeing or if we have any offers which we feel would benefit you to know about. We process your data to enable us to: form a contract with you to provide services, to serve our legitimate business interests, such as processing a payment or sending you a vaccination reminder, also where we have your consent such as for marketing. We may share your information with certain categories of third parties, to enable us to provide our products and services. These include: payment processors and reminders services such as Sumup, SMS Client Reminders, insurance providers, indemnity providers (Veterinary Defence Society), other professional and legal advisors, debt recovery agencies, referrals to other vets, specialist clinics and laboratories or other providers who support us in providing veterinary services to your horse(s). *
